This form is intended for requesting support for activities of the NVTG working groups that align with the goals of the Knowledge Center Global Health (KCGH).

KCGH assesses the applications based on a set of criteria incorporated in the application form. These criteria stem from agreements made with the Ministry of Health, Welfare, and Sport and are directly related to our objective.

Financial contributions can be disbursed to third parties, but these cannot be private individuals. Individual NVTG working group members are considered volunteers/private individuals, even if they are self-employed. They cannot receive a direct financial contribution from KCGH for the hours they dedicate to an activity.

Also, please note that the activity must take place in 2022.

KCGH aims to respond to requests within four weeks.

1. Wat is de titel van jullie activiteit? *

2. Wat is de naam van jullie werkgroep en wie is namens de werkgroep contactpersoon? Vul graag diens volledige naam, e-mailadres en telefoonnummer in.*

3. Geef een algemene omschrijving van de activiteit, inclusief de global health onderwerpen die aan bod komen.*

4. Wat is het doel van jullie activiteit en welke doelgroep(en) willen jullie bereiken?*

5. Welk meetbaar resultaat beogen jullie? *

6. Hoe draagt jullie activiteit bij aan kennisuitwisseling op het gebied van global health in Nederland? *

7. Hoe ziet de begroting van jullie activiteit eruit? Kunt u een duidelijk beschrijving sturen van de bedragen en waarvoor zou u ze gebruiken? Het is ook mogelijk deze te mailen naar info@kcgh.nl, bijvoorbeeld in Excel. Let op: begrote bedragen zijn incl. BTW **

8. Hoe ziet het dekkingsplan eruit? Geef aan welke financiële bijdrage jullie van Kenniscentrum Global Health vragen en of er sprake is van een eigen bijdrage/bijdrage van anderen. Het is ook mogelijk het dekkingsplan te mailen naar info@kchg.nl.*
